Request RMA

To request a Return Merchandise Authorization, complete this form and provide all required
information, including the model number, serial number, and purchase order number from the
original order.
Equipment may only be returned within the first 30 days after shipment. Other merchandise
may be returned within 90 days after shipment. Items older than 90 from shipment are not
eligible for return. If a return is approved, we will issue a Return Merchandise Authorization
(“RMA”) number.
Customer is required to return approved items within 14 days of receipt of the RMA number.
Damaged items and items that have been modified by customer are not eligible for return. If the
wrong item was ordered, the return is subject to a 25% restocking charge.
